How it works

Counting is done by computer vision, not by eye: cells are found in every frame and reduced to metrics. The count repeats the same way from test to test — which is what makes tests comparable instead of each one being read on its own.

What it is

Red cells stick to each other side by side and form stacks — “rouleaux”. The bond is reversible: plasma proteins build it, flow breaks it apart.

What it shows

How pronounced the stacks are depends on fibrinogen and other acute-phase proteins. This is the very aggregation behind the erythrocyte sedimentation rate — except you see it in the frame at once instead of an hour later in a tube.

What it rests on

The mechanism has been described since Fåhræus and is reproduced in ESR reviews. Its practical use is watching the inflammatory background over time; a single frame is not a diagnosis.
